Additionally, they will help devise any provider report cards and/or program adjustments.
Responsibilities Creates, analyzes, interprets, and distributes reports related to the respective programs the individual is supporting.
Keeps abreast of industry changes and serves as a subject matter expert on programs affecting quality, population health, and incentive programs.
Trains providers and clinics staff on measure specifications and quality improvement techniques.
Performs process improvement analysis and prepares/presents related documents for health care clinics.
Conducts workflow analysis and process mapping to identify areas for improvement.
Implement performance improvement tools to support clinic outreach activities, track data gaps, and decrease health care disparities.
Supports the Quality Department in service, process, and quality improvement efforts in support of lines of business and programs to ensure standardization and integration between programs.
Analyzes external health industry benchmark data to determine performance in access to care, quality, and patient experience.
Creates summary and detailed reports and presentations for leadership to support business and strategic decisions.
Performs other duties as assigned.
Works under the general supervision of the Director.
Conduct patient outreach as directed on topics related to member satisfaction, member experience and population health.
Maintain accurate knowledge of Hedis/Star ratings, CMS regulations, Medicare and general population health guidelines specifically as they pertain to patient satisfaction.
Utilize available tools to research patient concerns Experience Knowledge in Value Base quality metrics Ability to plan, prioritize and stay organized Excellent phone etiquette, strong communication, and customer service skills Microsoft Office, including Excel, Word, & Teams Primary Care office experience Experience in customer service or telephonic role Coordinate member outreach utilizing telephone, email, and text messaging.
Requirements/Skills Medical Assistant background Knowledge of medical terminology Social perceptiveness and service oriented Excellent customer service skills A high level of professionalism Excellent oral and written communication skills Excellent time management skills and ability to multi-task Proficient in MS Office and patient management software Time management Reports to Director of Quality Improvements
