As our Hospice Chaplain, you will play a critical role in providing essential support and guidance to our patients and their families in their time of need.
Responsibilities Providing spiritual and emotional support to hospice patients and their families Assessing the spiritual and emotional needs of patients and creating individualized care plans Collaborating with interdisciplinary teams to provide a comprehensive approach to care Offering bereavement support to family members after the passing of a loved one Documenting patient visits and care provided in accordance with company policies and procedures Participating in interdisciplinary team meetings and providing input for patient care Supporting the company's mission and values by maintaining a positive and professional demeanor Requirements Master's degree in Divinity or Theology Ordained or endorsed by a recognized faith group or equivalent experience Minimum of one-year hospice chaplain experience preferred Ability to work effectively with patients and families with diverse beliefs and backgrounds Excellent communication and interpersonal skills Ability to maintain confidentiality and work in a highly sensitive environment Strong organizational and time management skills.
