We are seeking a physician who is board certified in internal medicine (IM) or family medicine (FM) with a passion to serve older adults, who enjoys working with patients in all levels of post-acute care including LTAC, sub-acute rehab, and inpatient rehab.
Responsibilities:
Work alongside pharmacists on medication management Complete Medicare required visits Complete acute, follow up, and discharge visits as necessary Implement an individualized plan of care for patients' goals In collaboration with the IDT determine transitional care Make recommendations on DME, home health services, or outpatient services after short stay At TeamHealth, our purpose is to perfect our physicians' ability to practice medicine, every day, in everything we do.
