Senior Social Worker (Community Resource & Referral Center)

This professional Senior Social Worker is responsible to provide a variety of services to Veterans experiencing homelessness, who are unstably housed and/or at imminent risk of homelessness. This Senior Social Worker completes psychosocial assessments, treatment planning, short-term solution focused counseling with individuals and families who may be in crisis, education and referral for resources and benefits, case management and may provide post-intervention follow-up. The CRRC Senior Social Worker collaborates with interdisciplinary teams in the provision of comprehensive health care services (physical and/or mental health) to Veterans and ensures equity of access, service, and benefits to this population. The Social Worker collects, develops, and analyzes data using appropriate clinical indicators and monitoring activities to assure that the care provided is of the highest quality. The Social Worker provides consultation and education to VA staff and community based service providers including medical residents and Social Work students. The Social Worker documents Social Work activities and interventions in Veteran's clinical record utilizing the Computerized Patient Record System (CPRS) and ensures appropriate hand off when transferring of Veteran care. Responsibilities will also include the collection and maintenance of statistical data, completing and monitoring information in the VA Homeless Operations Management System (HOMES), program planning evaluation and modification, and the supervision of students as assigned. The CRRC Senior Social Worker may work days/tour hours that are outside of normal business hours, with limited or no on site supervision. The Social Worker independently provides clinical psychosocial treatment and case management services at an advanced practice level to eligible Veterans and their family members to meet biological, psychological, social and environmental needs. Veterans treated come from a wide geographic area. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving. The CRRC Senior Social Worker may participate in specialized training required for Social Work Services Interdisciplinary staff. The Social Worker acts as the subject matter expert on VA and/or community resources. The Social Worker will collaborate with other service providers in assessing the Veteran's needs and linking them to the appropriate resources and/or programs.The Social Worker is responsible for educating the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker. The Social Worker will serve as a liaison with community agencies, assist Veterans with referrals for services from community and government agencies, and help coordinate these services. Services may be provided in the community (for example):
agencies, residences, shelters and places not meant for habitation (i.e. cars, deserts, parks, streets, and/or tunnels/storm wash areas). The CRRC Senior Social Worker may spend a significant portion of work time participating in outreach to Homeless Veterans in the community. This may include completing assessments / visits with Veterans in rural areas and areas not meant for human habitation (i.e. parks, deserts, tunnels, etc). Outreach may also take place in conjunction with other community agencies, for example Stand Down and Point in Time (PIT) Count events. The CRRC Senior Social Worker understands the intimidation of bureaucracy and will act as an advocate within the VA system when it serves the best interest of the Veteran/family. When appropriate and feasible, Social Worker will educate and encourage the Veteran/family to advocate on his / her own behalf, thus fostering a sense of independence and empowerment. The CRRC Senior Social Worker is experienced in making rapid assessments and developing crisis management plans for admission to mental health inpatient programs. Social Worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. In the event of a medical or psychiatric emergency, staff will follow the protocol of the VA Medical Center. This may include assessment of suicidality / homicidality and initiation of Legal 2000 paperwork needed for emergent psychiatric evaluation. Other additional duties as assigned. Work Schedule:

Physical Demands of the work are generally minor. The work is primarily sedentary, although the stress involved in dealing continually with persons in emotional distress and who may be uncooperative and occasionally violent makes this work notably more demanding than most sedentary jobs. Qualifications:
Citizenship. Be a citizen of the United States. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determination:
GS-12 Senior Social Worker Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities 1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex physical or mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority for services and provide specialized treatment services. 2. Advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with polytraumatic injuries, spinal cord injuries, traumatic brain injuries, visual impairment, post-traumatic stress disorder, etc. 3. Advanced knowledge and expert skill in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on empirical findings. 4. Ability to provide subject matter consultation to colleagues and students on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Ability to teach and mentor staff and students in the specialty area of practice and to provide supervision for licensure or for specialty certifications. 5. Ability to expand clinical knowledge in the profession, demonstrating innovation in the creation of new models of psychosocial assessment or intervention to identify and address specialized clinical needs. Ability to write policies, procedures, and/or practice guidelines pertaining to the specialty population or specialty treatment program. Tour of Duty:
